Housing First Assertive Community Treatment Team (ACTT)
Our Housing First Assertive Community Treatment Team combines the evidence-based ACT and Housing First models and serves adults with serious mental illness who have experienced chronic homelessness. We provide a full range of treatment, rehabilitation and support services delivered by an interdisciplinary team in a variety of community based settings. Services include psychiatric and nursing services, assistance with housing and employment, substance abuse treatment, peer support and case management. All services focus on preventing homelessness.
ACTT is for Adults, 18 years old or older in Guilford County.

Hours Of Operation:
Hours are Monday through Friday 8 am to 4 pm and on-call for emergencies 24 hours a day, 7 days a week.
